HCV-NS3 protein (genotype matched) was incubated with purified cryoprecipitated IgM fraction from two patients. The patients have type II mixed cryoglobulinemia (MC) and positive for anti-HCV antibodies and HCV RNA. After trypsin digestion, IgM bound peptide epitopes were dissociated and subjected to MALDI-TOF analysis. Purified IgM from 8 out of 17 patients with HCV positive MC were reactive against HCV NS3 protein, but not E2, core, NS4A, NS4B and NS5 HCV antigens, in ELISA and immunoblot. HCV positive type II MC patients have significant higher cryocrit value (cryoglobulin level) than HCV negative MC patients. All cryoprecipitated IgM have reactivity against human IgG and its Fc fragment by ELISA. HCV NS3 antigen inhibited binding of purified IgM from these two patients to human IgG and, vice versa, IgG Fc fragment inhibited binding to NS3 antigen.
