Role of minor determinants of amoxicillin in the diagnosis of immediate allergic reactions to amoxicillin.

Of 61 amoxicillin-allergic patients, 36 reacted to the epitope by prick and intradermal testing. This included 9 of 18 patients that were also positive to one or more of the determinants penicilloyl-polylysine, minor determinant mixture formed by benzylpenicillin, benzylpenicillin, and amoxicillin, and 27 of 33 that were positive only to amoxicillin. Ten other patients were skin test negative for all determinants, but were positive in a drug provocation test with amoxicillin.
